envisionmd Masterfile
In an effort to move toward a paperless work place the envisionmd team has gone to great lengths to offer an electronic means of submitting both PM and EMR Masterfiles. In an effort to streamline the submission of Masterfile requests, we have uploaded all Masterfiles to the Saint Francis HealthCare Partners website, allowing all users the ability to complete request forms from their work station and eliminate the need to fax documents. It is our anticipation that this new means of Masterfile submission will not only save you time and filing space, but we also allow for faster data entry and turnaround times by our team.
We ask for your help in making this transition to electronic request submission a success by following the steps below. Our anticipation is to eliminate faxed submission of Masterfiles by March 1, 2012.

- Select and open the appropriate Masterfile Form from SFHCP website http://www.sfhcp.org/
- After the document has been filled out in its entirety (some fields are required) click SUBMIT.
- Once the request has been completed, someone from the envisionmd team will send you an email confirmation upon completion.
