NCQA Recognition Programs

The National Committee for Quality Assurance (NCQA) develops quality standards and performance measures for a broad range of health care entities. The measures and standards are used by organizations and individuals to identify areas for improvement.

NCQA offers Physician Recognition Programs that allow physicians to assess individual performance against nationally recognized, evidence-based clinical standards of care.

Achieving NCQA recognition awards the physician the distinction of being identified as a high quality physician who is a leader in delivering quality health care. More than 10,000 of the nation's leading physicians and practices have earned this distinction. Recognized physicians are listed online in the NCQA Recognized Physician Directory and are specially denoted in the Saint Francis Physician Directory.

Clinical Integration Initiative Promotes Recognition Process

Saint Francis HealthCare Partners (SFHCP) manages the recognition process at no cost to physician members or practices as part of the Clinical Integration Program. SFHCP's Care Coordination staff visits the physician's office or practice, abstracts all necessary information, analyzes the data to ensure recognition eligibility and provides feedback. If required standards are met, SFHCP completes the NCQA application process, pays the application fees and submits the information.

When a physician or practice receives NCQA recognition, SFHCP frames the official NCQA certificate and presents it to the recipient.

SFHCP offers all eligible physicians assistance in obtaining recognition in the following NCQA Recognition Programs:

PPC-Patient-Centered Medical Home (PCC-PCMH)
Recognizes physician practices functioning as medical homes by using systematic, patient-centered and coordinated care management processes.

Back Pain Recognition Program (BPRP)
Recognizes physicians who take a patient-centered approach and deliver care in accordance with widely accepted, evidence-based guidelines.

Diabetes Recognition Program (DRP)
Recognizes providers who demonstrate that they provide high-quality care to patients with diabetes.

Heart/Stroke Recognition Program (HSRP)
Recognizes physicians who demonstrate that they provide high-quality care to patients with cardiac conditions or who have had a stroke.

Physician Practice Connections (PPC)
Recognizes physician practices that use up-to-date information and systems to enhance patient care.
